Downhill: The Building of Fast Moving and Healthy Startups

I started a list of startups that were famous once, and crashed and burned.  The list was enormous.  Colorful stories were all that was left from a few.  Boo.com, WebTV, Startup.com, eXcite, Razorfish, WebVan.com and on and on.  Based upon my book, I think most people agreed that these startups all had great ideas.  Venture capital poured more money into one or two of these, than I would need in a startup lifetime.  So what happened?

Idea before its time?  Bad culture and leadership?  Poor execution?  Terrible messaging?  Failed sales?  Not sure.

But I would like to here stories (which you can protect the names of the innocent) that provide lessons of what to NOT do or what TO do.  What is it that a startup can do to make it?
